LWM 45 and the DHS Central Registry Clearance forms must be filled out and returned to Living Water Ministries by December 10th, 2011. *Individuals who submit LWM 45 or the DHS Central Registry Clearance forms after December 10th, 2011 will be charged a $10 processing fee for each.
